نظریه‌&shy;های کارشناسی پزشکی صحیح و متقن در خصوص پرونده‌های افراد محجور ناشی از دمانس، این مطالعه با هدف بررسی میزان توافق (پایایی) نظرات صادره توسط ادارات کمیسیون پزشکی استان‌ها با کمیسیون پزشکی مرکزی و اثر استفاده از معیارهای تشخیصی مانند آزمون روان‌شناختی (</span><span dir="LTR">MMSE</span><span lang="FA">)، وجود نتایج یک تصویربرداری عصبی، مشاوره&shy;‌های خارج سازمانی و حضور نورولوژیست در جلسات کارشناسی بر میزان این توافق، صورت‌</span> <span lang="FA">گرفت.</span></span></span><br> <span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA">روش‌ بررسی:</span></b> <span lang="FA">بررسی حاضر روی ۱۲۲ پرونده</span> <span lang="FA">افراد مدعی محجوریت ناشی از دمانس ارجاع شده به اداره کمیسیون پزشکی مرکزی در فاصله شهریور سال ۱۴۰۱ تا شهریور سال ۱۴۰۲ انجام گرفته است</span><span dir="LTR">.</span><span lang="FA"> برای بررسی میزان توافق نظرات صادره از ضریب کاپای کوهن، جهت تحلیل داده&shy;‌ها از روش&shy;‌های آمار توصیفی و برای بررسی رابطه بین بعضی از معیارهای تشخیصی بر میزان توافق صورت گرفته از رگرسیون لجستیک با استفاده از نرم</span><span dir="LTR">&shy;</span><span lang="FA">افزار </span><span dir="LTR">SPSS</span><span lang="FA"> ویرایش ۱۹ استفاده شد.</span></span></span><br> <span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA">یافته‌ها:</span></b><span lang="FA"> بین نظریه‌&shy;های کارشناسی صادره در خصوص محجوریت یا عدم محجوریت در آخرین کمیسیون برگزار شده در استان‌&shy;ها و اداره کمیسیون مرکزی توافقی خوب با ضریب کاپا ۰.۶۲۶ و در خصوص زمان شروع حجر بین آنها توافقی خوب با ضریب کاپا ۰.۶۸۱ وجود دارد. در ۷۶.۲ درصد پرونده‌ها آزمون </span><span dir="LTR">MMSE</span><span lang="FA">، ۷۷.۸ درصد مشاوره&shy;‌های خارج سازمانی، ۲۴.۶ درصد تصویربرداری عصبی یا نتایج مربوط به آن وجود نداشته است. در صورتی که این سه متغیر دارای اثر معنی&shy;‌دار و مثبتی بر میزان توافقات صورت گرفته دارند. </span><span lang="FA"><span style="background:yellow"><span style="font-family:"B Mitra""></span></span></span></span></span><br> <span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA">نتیجه‌گیری:</span></b> <span lang="AR-SA">علیرغم </span><span lang="FA">اینکه میزان توافق در نظریه‌&shy;های کارشناسی صادره بین آخرین کمیسیون برگزار شده در استان و کمیسیون مرکزی خوب است، اما زمانی می‌&shy;توان بر توافق بین ارزیاب‌&shy;ها به‌عنوان یک منبع قابل‌اطمینان تکیه کرد که ارزیابی‌&shy;ها باتوجه‌ به معیارهای تشخیصی مشخص و استانداردی صورت گرفته باشد؛ لذا استفاده از یک دستورالعمل استاندارد و واحد، به‌کارگیری ابزارهای روان‌سنجی جدید برای تشخیص دقیق‌تر ظرفیت ذهنی بیماران و آموزش مداوم روانپزشکان و روان‌شناسان ضروری می‌&shy;نماید.</span></span></span></span></span></span></span></div> <div style="text-align: justify;"><span style="color:#222222;"><span style="font-size:14px;"><span style="font-family:Times New Roman;"><span style="line-height:normal"><b><span style="background:white">Introduction:</span></b> <span style="background:white">Considering the importance of issuing correct and valid legal medical expert opinions about the financial incapability due to dementia,</span> <span style="background:white">This study aims to examine the level of agreement between the expert opinions issued by the provincial legal medicine commission offices and the central legal medicine commission office and the effect of the use of diagnostic criteria such as the psychological test (MMSE), the existence of the results of neuroimaging, external consultations and the presence of a neurologist in expert meetings has been done on the level of this agreement.</span></span><br> <span style="line-height:normal"><b><span style="background:white">Methods:</span></b><span style="background:white"> The present study was conducted on 122 cases of people claiming financial incapability due to dementia referred to the central medical commission from September 2022 to September 2023.</span> <span style="background:white">To examine the level of agreement (reliability) of expert opinions issued by Cohen&#39;s kappa coefficient, to analyze data from descriptive statistics methods and to investigate the relationship between some diagnostic criteria on the level of agreement, logistic regression was used using SPSS 19 software.</span></span><br> <span style="line-height:normal"><b><span style="background:white">Results:</span></b><span style="background:white"> There is a good agreement with the Kappa coefficient of 0.626 between the expert opinions</span> <span style="background:white">issued regarding the financial incapability or the financial capability in the last commission held in the provinces and the Central Commission, and there is a good agreement with the Kappa coefficient of 0.681 between the expert opinions issued regarding the time of onset of financial incapability. In 76.2% of cases, there was no test MMSE, 77.8% of external consultations, 24.6% of neuroimaging or related results.
