بررسی رابطه بین یافته های بالینی و نتایج پاتولوژی در آپاندیسیت حاد کودکان در بیمارستان حضرت علی اصغر (ع)

چکیده
زمینه و هدف: آپاندیسیت از بیماری های مهم و شایعت رین اورژانس کودکان است. با توجه به سرعت سیر بیماری و دشواری تشخیص،
روش های گوناگون از جمله استفاده از امکانات پاراکلینیکی- همچون سونوگرافی، رادیولوژی و سیت یا سکن جهت تشخیص به موقع و انجام
جراحی و جلوگیری از عوارض ناخوشایند بیماری، در مراکز مختلف مورد استفاده میب اشد. نظر به عدم دسترسی به این امکانات و یا گرانی
آن در برخی مراکز، ارزیابی علایم بالینی و نتایج معاینات در تعیین شدت بیماری اهمیت میی ابد. در این مطالعه، علایم بالینی کودکانی که با
تشخیص آپاندیسیت حاد تحت عمل جراحی قرار گرفتها ند با نتایج پاتولوژی آنها تطبیق داده شده است. با توجه به نتایج بررسی به نظر می رسد
با ارزیابی علایم بالینی بتوان تا حدود زیادی میزان پیشرفت بیماری را از لحاظ پاتولوژی حدس زده، تصمیم لازم را برای انجام عمل جراحی
اتخاذ نمود.
روش بررسی: موارد بررسی 58 کودک (دختر و پسر) در طیف سنی 5/ 3 تا 14 سال بودند که با تشخیص اولیه آپاندیسیت تحت عمل جراحی
قرار گرفته بودند. پرونده های بیماران به صورت گذشتهن گر مورد بررسی قرار گرفت. علایم و نتایج معاینات بالینی با نتایج پاتولوژی تطبیق داده
شد.
یافته ها: این بررسی مشخص نمود که نشانه های بالینی و یافته های معاینات با نتایج پاتولوژی تطابق کامل دارند و به تناسب پیشرفت و شدت
نشانه ها، یافته های پاتولوژی آپاندیسیت نیز متناسب با آن پیشرفت داشته است.
نتیجهگیری: در صورتی که امکانات پاراکلینیکی همچون سونوگرافی و سیت یا سکن در دسترس نباشد، میت وان در کودکان مشکوک به
آپاندیسیت حاد پس از اخذ شرح حال دقیق و انجام معاینات بالینی، تا حدود زیادی تشخیص را سرعت بخشید و با تصمیم به موقع برای عمل
جراحی از پیدایش عوارض ناخوشایند جلوگیری به عمل آورد
Abstract :
Background & Aim: Acute appendicitis is one of the commonest medical emergencies in children. Due to rapid
progression of the disease and difficulty in diagnosis, different paraclinical methods including ultrasonography,
radiology and CT scan etc, have been used in order to diagnosis the disease at early stage and thus prevent from
any related complications. However, because of unavailability of the above mentioned paraclinical procedures in
certain areas and / or it’s high cost, evaluation of clinical manifestation is very important in determining the severity
of the disease. In this study the clinical manifestation of children, who had undergone surgery with the diagnosis of
acute appendicitis, was compared with the pathological report. In this way by everything the clinical features, the
progressing of the disease could be estimated, to large extent by pathology and thus decision in regard to surgery
would be taken
Materials and Methods: In this study 58 children (including both girls and boys) between the ages of 3.5 year – 14
with the primary diagnosis of acute appendicitis underwent surgery. As a retrospective study, the medical files of the
patients were rewied and the clinical features were compared with the pathological reports.
Findings: Clinical manifestation and the pathological reports were comparable with each other. according to
results, increase in security of clinical features was associated with more rapid progression of pathology.
Conclusion: In case of unavailability of Paraclinical methods such as ultrasonograohy and CT-scan more
attention must be paid to clinical history and clinical sign and symptoms observe in a child suspected of having acute
appendicitis. By this method, early diagnosis could be accelerated and any decision in regard to surgery would be
taken accurately .As a result of unpleasant complications are prevented
