fa بررسی و ارزیابی وضعیت بیماران مسموم اعزام شده از سایر مراکز درمانی و اقدامات درمانی انجام شده قبل و حین اعزام در اورژانس مسمومین بیمارستان نور اصفهان Evaluation of the Therapeutic Interventions Performed in Patients Before and During Referral to Poisoning Emergency Department in Isfahan Province پزشکی قانونی Forensic Medicine مقاله پژوهشی Research Article مقدمه: مسمومیت یکی از شایعترین اورژانس های پزشکی است. در مراکز درمانی، اقدامات درمانی لازم جهت این بیماران به فراخور امکانات و سطح آگاهی پزشکان انجام میشود. اینجاست که موضوع اعزام بیماران مسموم به مرکزی که دارای پزشکان متخصص و با تجربه در زمینه مسمومیتها و همچنین امکانات کافی باشد پیش میآید. مطالعه حاضر برآن است تا وضعیت اعزام این بیماران را ارزیابی نماید. روش اجرا: این مطالعه، یک مطالعه توصیفی گذشتهنگر و جامعه مورد مطالعه آن بیماران مسموم اعزام شده در طی سال از مراکز درمانی سطح استان اصفهان به اورژانس مسمومین بوده است. روش نمونه گیری بصورت سرشماری بوده و محقق پرسشنامه را براساس پرونده بیمار ارجاعی تکمیل نموده است. نتایج: حجم نمونه ۳۶۳ مورد در طی یک سال بود. ۵۳% (۱۵ نفر از۳۲ نفر) بیماران نیازمند به انتوباسیون انتوبه نشده بودند. ۲۷% (۹۸نفر) بیماران اعزامی همودینامیک ناپایدار داشتهاند که جهت پایدار کردن 68% (۲۵ نفر) آنها اقدام انجام شده و این اقدام تنها برای ۵۹% (۱۴ نفر) آنها مؤثر بوده است. ۱۳% (۴۷ نفر) کل بیماران اعزامی در مرکز اعزام کننده در کما بودهاند و در ۲۴% (۱۱ نفر) بیماران کوکتل درست تجویز شده بود. سمزدایی با شستشوی معده در ۸۷% بیماران نیازمند (۲۶۷ نفر از ۳۰۶ نفر) به آن انجام شده که در ۶۲% (۱۶۵ نفر) مؤثر بوده است. تنها برای ۸% بیماران نیازمند به شارکول ( ۱۵نفر از ۱۹۶ نفر) این ماده تجویز شده است. در بخش مسمومین برای 63% (۲۲۹ نفر) بیماران اعزامی اقدام تخصصی درمان مسمومیتها انجام شده است. 95% پزشکان اعزام کننده پزشک عمومی بودهاند که آموزش دوره مسمومیتها را ندیده بودند. شایعترین علت اعزام (36% موارد) نبودن تجهیزات درمانی بود. نتیجهگیری: به منظور کاهش مشکلات اعزام بیماران مسموم باید بیشترین توجه بر افزایش آگاهی پزشکان در دوره طب عمومی با گذراندن بخش مسمومیتها و تاکید بر حضور فارغ التحصیلان در برنامه های آموزشی مسمومیتها انجام شود. لازم است بر مجهز بودن مراکز درمانی به وسایل و داروهای لازم مانند کوکتل کوما و شارکول نظارت کافی شود Background: Poisoning is one of the most common medical emergencies. The present study is conducted to determine the condition of transferring poisoned patients from medical centers to Noor Poisoning Center in Isfahan province of Iran. Methods: The present study is a descriptive-retrospective study. The studied population has been the patients who were transferred from medical centers to the poisoning center during a year. The sampling has been conducted in census method, and the researcher designed the questionnaire with regards to the available data in transfer forms which held questions about the preliminary measures taken in the management of the poisoned patient. Findings: the population involved 363 patients who were transferred to our referral center during one year. About 53% (15 of 32 cases) of the patients, who had required intubation, were not intubated prior to transfer to the poisoning emergency room. About 27% (98 cases) of the dispatched patients had unstable hemodynamics, and stabilization measures <a name="OLE_LINK20"></a><a name="OLE_LINK19">had been performed for </a>68% (25 cases) of the clinically unstable patients. Such measures were beneficial in 59% (14 cases) <a name="OLE_LINK16"></a><a name="OLE_LINK15">of these patients</a>. About 13% (47 cases) of the patients were comatose and an appropriate prescription of coma cocktail <a name="OLE_LINK14"></a><a name="OLE_LINK13">had been done </a>in 24% (11 <a name="OLE_LINK49"></a><a name="OLE_LINK48">cases</a>) of them. Around 87% (267 of 306 cases) <a name="OLE_LINK18"></a><a name="OLE_LINK17">of the patients</a> requiring gastric lavage had been lavaged which had been efficient in 62% (165 cases) of them. Charcoal had been only prescribed for 8% (15 of 196 cases) of the patients who required it. Special poison treatments had been administered in 63% (229 cases) of patients. In addition, 95% of the referring doctors were general practitioners who had not passed the poisoning course. The most common reason for transferring, according to the forms data, had been the lack of appropriate medical facilities (36% cases). Conclusion: Apparently, in order to reduce the problems of transferring poisoned patients, the most attention should be paid to increasing awareness of the physicians in general medicine course by including a poisoning course and also by emphasizing the presence of graduates in training poisoning programs.
