fa مقایسه نتایج آرتروسکوپی و MRI زانو در آسیب های تروماتیک زانو Comparison of Results of MRI & Arthroscopy in Traumatic Knee Injuries پزشکی قانونی Forensic Medicine مقاله پژوهشی Research Article زمینه و هدف: ترومای زانو یکی از شایع ترین مواردی است که لازم است توسط متخصصین پزشک قانونی مورد بررسی قرار گیرد. در این مطالعه قابلیت اعتماد و ارزش MRI و معاینه بالینی در ارزیابی ضایعه منیسک ها و رباط متقاطع قدامی و خلفی در بیمارانی که به دلیل ترومای زانو به بخش معاینات بالینی پزشکی قانونی یزد مراجعه کرده بودند مورد بررسی قرار گرفت. مواد و روش ها: در این مطالعه 114 بیمار که به دلیل شکایت از ضایعه ترومایک زانو متعاقب تروما از آبان 1392 لغایت اردیبهشت 1393 مورد MRI و آرتروسکوپی قرار گرفته بودند، وارد مطالعه شدند. با در نظر گرفتن نتایج آرتروسکوپی به عنوان استاندارد طلایی، حساسیت، ویژگی، ارزش اخباری مثبت و منفی و دقت تشخیصی MRI و معاینه بالینی در این بیماران ارزیابی شد. یافته ها: بر اساس نتایج آرتروسکوپی، پارگی کامل رباط متقاطع قدامی (42/1%) و پارگی ناکامل رباط های متقاطع قدامی و خلفی (هر کدام در 26/3%) شایع ترین عوارض گزارش شده بودند. این مطالعه نشان داد دقت تشخیصی MRI برای ضایعات رباط متقاطع قدامی، رباط متقاطع خلفی، منیسک داخلی و منیسک خارجی به ترتیب 68/4%، 81/6% ، 71/9% و 99/1% بوده است. از سوی دیگر دقت تشخیصی معاینه بالینی برای ضایعات رباط متقاطع قدامی، رباط متقاطع خلفی، و منیسک ها به ترتیب 59/6%، 78/9% و 75/4% بوده است. نتیجه گیری: مطالعه حاضر نشان داد نتایج یافته های معاینه بالینی و MRI زانو الزاماً با نتایج آرتروسکوپی انطباق ندارند. هم چنین با توجه به نتایج مطالعه حاضر به نظر میرسد تکیه بر یافته های معاینات بالینی، به منظور ارزیابی ضایعات تروماتیک زانو در بررسی های پزشکی قانونی کافی نیست. Background: Traumatic knee injuries are among the most common cases that should be evaluated by a forensic medicine specialist. The present study examined the accuracy and reliability of MRI and clinical examination in evaluation of meniscal and anterior and posterior cruciate ligaments in subjects who referred to the Yazd clinical forensic medicine ward depatrment. Methods: In this study, 114 patients with complaint of traumatic knee injury who had undergone arthroscopy and MRI between October 2013 and May 2014 were included. Considering arthroscopy results as a gold standard, sensitivity, specificity, positive and negative predictive value, and accuracy of MRI and clinical examination were evaluated. Findings: According to arthroscopic investigations, complete tearing of the anterior cruciate ligament (42.1%) and partial tearing of the anterior and posterior cruciate ligaments (26.3% each) were the most common reported complications. This study indicated that the accuracy of MRI for anterior cruciate ligament, posterior cruciate ligament, medial meniscus and lateral meniscus lesions were 68.4%, 81.6%, 71.9%, and 99.1% respectively. Moreover, the accuracy of clinical examination for anterior cruciate ligament, posterior cruciate ligament, and meniscal lesions were 59.6%, 78.9%, and 75.4% respectively. Conclusion: The present study indicated that knee MRI and clinical examination results are not obligatory in concordance with arthroscopic findings.
